What kind of router are you using?
I had trouble forwarding ports properly on my BeBox (thomson tg585v7), something to do with "PAT" I think (Port Address Translation).
I ended up using a cheap buffalo router, flashing it to use DDWRT firmware, then putting my BeBox router in bridged mode. All my port forwards could then be done on the buffalo router and the BeBox just sat there as a gateway.
That "might" be your problem...or..
Perhaps your router is not allowing loopback connections, e.g. your friends can see your server because they are outside the LAN, but you cant.
