Because of the special case routing needed. Packets coming in on the internal interface won't be subject to forwarding and therefore are dropped at the router. Simply forwarding packets on the internal interface will break connecting to any other server than your own. So you need a router that takes internally received packets and apply forwarding like ti came in on the external interface.
Most stacks don't do this.
